Consent Light / Energy Based Laser Treatment

I authorize The Natural Place Med Spa to perform:

Photo Facial, Rejuvenation or IPL (3 weeks) – Intense Pulsed light treatment refers to a cosmetic skin treatment that uses high intensity light to address certain imperfections of the skin-including facial hyperpigmentation and discolorations (rosacea, melasma, freckles, sun, and age spots), superficial vascular blemishes, wrinkles and acne scars. (Refer to laser post care prior to service)

Laser Hair Removal (2-4 weeks) – Shave area same day shaving is recommended, Ranges from 2-week intervals depending on body part. (Refer to laser post care prior to service)

Spider Veins Laser – (3 weeks) IPL is used to treat spider veins, also known as telangiectasias, the light generates heat in the veins. This causes the walls of the vein to collapse and form scar tissue. Eventually, these veins will be reabsorbed by the body and fade away after several weeks. (Refers to laser Tx pre/post care)

Pico Lazer Qswitch Tech/Fractional Laser – Tattoo Removal, Melasma, Wrinkles, Acne & Scar Reduction (2weeks-8weeks) –

This homogeneous beam delivery energy spreads evenly over the skin’s surface, minimizing epidermal injury and increasing patient satisfaction. Q switch removes tattoos, treats pigmentary conditions, and reduces acne scarring and wrinkles.

Toe Nails Fungal TX By Laser: (4weeks) Post treatment you should apply an Prescription antifungal liquid massaging onto infected toes/feet morning and night. You should use Dial Gold Soap to cleanse area of bacteria, keep nails cut and trimmed and a yeast and fungal supplement w/ a pre & pro biotic dairy free might be recommended if nails have gone untreated. Do not discontinue their use; this will put you at greater risk of reinfection. (More instruction will be provided @ consultation time as every client is as individual)

By light/energy system is intended to correct change current condition. Please note than more one session may be necessary to achieve desired result. However, other treatments, including skin care products, are often needed to blend color, reduce sun damage, and give the best results.

The skin treated may be red and swollen with fine, thin scabs forming. Keep in compliance with aftercare instruction for best results. The healing process can take anywhere from 1-3 weeks. It could take as long as 3-6 months in some rarer cases.

Please do know we approved our BODY BRONZE spray tanning as other brands will forfeit your voucher, as we do not promote pre-tanning of any kind on area site. Please reschedule or wait ‘till after vacations, amid or events. As this will result of injured skin or adversities.

Pre-Post Care: Unprotected sun exposure or use of tanning beds, creams in areas to be treated. We might request rescheduling and or no treatment as you are taking a risk of injury, Pregnancy, Menstrual dysfunction, use of mechanical epilation less than 4-5 weeks prior to treatment for patients seeking hair removal, Allergies, History of Seizures, Medications, History of keloid and hypertrophic scar formation. Active infection/immunosuppression. Open lesion, Herpes 1 or 2 within treatment area, Tretinoin (Retin A or Renova), Oral isotretinoin (Accutane) within the preceding six months (Dr. Note will be required before Tx is started) No sun-tanning or self-tanners 8 weeks prior to treatment. Includes spray tans, taming lotions, tanning beds, sun exposure, etc. Avoid treatments that may irritate the skin for 4 weeks prior to treatment (waxing, depilatories, etc.) Notify clinic with any changes to your health history or medications since your last appointment. History of herpes or cold sores may require an anti-viral prescription prior to treatment if treatment in the area that breakouts occur.

Post-Laser Tx Only

A mild sunburn like sensation is expected. This usually lasts 2-24 hours but can persist up to 72 hours. Mild swelling and or redness may accompany this, but usually resolves in 2-3 days. Apply Aloe Vera and or ice-cold packs to the treatment area for 10-15min every hour for the next four hour, or as needed. An oral, non-steroidal anti Inflammatory, such as Acetaminophen may be taken to reduce discomfort. ln some cases, prolonged redness or blistering may occur, Aloe Vera may be applied and SPF 45/50 to affected area twice a day until healed.
